Deeply ingrained in all of our society and systems is the blind belief in Competition as the normal means of achievement. When the playing field isn't fair, Competition always favors the privileged. Understanding the real cooperative ability of human beings is difficult for doctors who believe they are 'winners' at everything because they 'fought hard' for it. In reality, nobody is equal, but everyone has value. Learning to abandon heroism as a mindset goes along with that.

Gatekeeping is another one of those aspects of medicine that needs to go away, both because of the racism and because of Ta Interwebs. Once we finally figure out that people need good health (not insurance), then everyone will have to become a participant in it, not just the aristocratic hierarchy and its white uniforms.
